I recently upgraded to OS 9.1, and in the process, it seemed I lost Note Pad. I couldn't believe Apple had killed this useful utility, but it was nowhere to be found in the Apple Menu Items folder. While it used to reside in the Apple Menu, it's no longer there. After a few moments' panic, I fired up Sherlock...which reported that it found Note Pad into the Applications (Mac OS 9) folder. If you prefer it in the Apple Menu, though, just drag it back to the Apple Menu Items folder. Whew!
